RMG/IVF Surgery Center - Advance Directives/Living Wills

We recognize your right to have Advance Directives and a Living Will according to the Federal and State of Florida laws and Statutes. We also recognize that the purpose of being an ambulatory surgery center is to provide elective surgical procedures for relatively healthy, stable patients. For this reason, RMG/IVF Surgery Center has elected not to recognize and/or provide patient care, as outlined in an advance directive and/or living will document, during the time you are being treated at the Surgery Center.

If you have a Living Will or Advanced Directives, please bring a copy with you. A copy of your Advance Directive will be made and retained in your medical record for reference. It will also accompany you in the rare event that you may need to be transferred to a hospital facility.
