What Is Clomiphene Citrate? 

Clomiphene citrate is an oral medication used to help induce ovulation in individuals who are not ovulating regularly or at all. It belongs to a class of drugs known as selective estrogen receptor modulators (SERMs). It works by blocking estrogen receptors in the hypothalamus, which signals the brain to increase production of key reproductive hormones, FSH and LH, that stimulate the ovaries to develop and release an egg. 
 

This mechanism helps promote the growth of ovarian follicles and supports predictable ovulation, which is crucial for timing conception. 

 

How Clomiphene Citrate Stimulates Ovulation 

When the hypothalamus senses low estrogen (because clomiphene blocks estrogen’s feedback), it responds by releasing more GnRH. This triggers: 

  • FSH (Follicle-Stimulating Hormone) → stimulates follicle growth 

  • LH (Luteinizing Hormone) → triggers ovulation 

This hormonal cascade helps the ovary mature and release an egg, creating the conditions needed for conception. 
 

 

When Clomiphene Citrate May Be Prescribed 

Clomiphene citrate is used in several common fertility situations, including: 

1. Irregular or Absent Ovulation 

Clomiphene is most successful as a first line therapy for individuals with inconsistent or absent menstrual cycles, such as those with polycystic ovary syndrome (PCOS). 
 

2. Unexplained Infertility 

Clomiphene can be used even when ovulation is normal, but pregnancy has not occurred despite regular cycles and no identifiable cause of infertility. 
 

3. As Part of Fertility Treatment Cycles 

Clomiphene may be used to: 

  • Increase the number of mature eggs for timed intercourse 

  • Assist in preparing for advanced reproductive treatments

 

4. Special Cases Where It May Not Be Effective 

Clomiphene is not typically effective for individuals who: 

  • Have very low body weight 

  • Experience hypothalamic amenorrhea 

  • Have primary ovarian insufficiency
