Standard clinical guidance recommends seeking a fertility evaluation after twelve months of unprotected intercourse without a resulting pregnancy, or after six months if the female partner is 35 or older. Earlier assessment may also be appropriate for men who have had:

What is a semen analysis, and why does it matter?
A semen analysis is often one of the first diagnostic tools used to evaluate male fertility. While relatively simple to perform, it provides extensive information on key aspects of sperm health.
During the evaluation, a laboratory technician examines the sample for the following:
- Sperm count: the number of sperm present in the sample
- Motility: the percentage of sperm that move correctly and effectively
- Morphology: whether sperm have a normal structure and appearance
- Volume: the amount of semen produced during ejaculation
Findings are evaluated against standardized reference ranges set by the World Health Organization (WHO), allowing your provider to determine whether or not any sperm parameters are abnormal and may be impacting fertility.
Patients are usually instructed to avoid ejaculation for two to five days before collection. Samples may be collected in a private room at the clinic or at home when appropriate, provided collection instructions are followed carefully.
What other male fertility tests might be recommended?
While semen analysis is often the foundation of male fertility testing, additional testing may be necessary when initial findings warrant further evaluation. Additional testing can help physicians investigate why certain abnormalities are present and whether additional factors are affecting reproductive function.
Medical History and Physical Examination
A fertility evaluation begins with understanding the broader medical context. Your physician may review previous illnesses, surgeries, medications, occupational exposures, family history, and reproductive history. Your physician will also conduct a physical examination to identify any structural abnormalities that may be impacting fertility.
Hormone Testing
Sperm production is regulated by a cascade of hormones, and if any are out of balance, the effects on fertility can be significant. Retrograde Semen Analysis
In some men, semen travels backward into the bladder during ejaculation rather than exiting through the urethra — a condition known as retrograde ejaculation. A retrograde semen analysis evaluates a urine sample collected after ejaculation to detect the presence of sperm, confirming whether this condition is present.
Semen Culture
A semen culture tests the semen sample for the presence of bacteria or other infectious organisms that could be affecting sperm function or reproductive tract health. Infections can contribute to inflammation, impaired sperm motility, and other fertility concerns.
