What are the signs of male infertility, and when should I get tested?

There are often no obvious signs of male infertility. For most men, the first indication that something may be affecting fertility comes only after months of trying to conceive without success.

Standard clinical guidance recommends seeking a fertility evaluation after twelve months of unprotected intercourse without a resulting pregnancy, or after six months if the female partner is 35 or older. Earlier assessment may also be appropriate for men who have had: 

  • Testicular injury or trauma
  • Prior reproductive surgery
  • Infection of the reproductive tract
  • Chemotherapy or radiation treatment
  • Known genetic conditions that impact fertility 

Men who are not actively trying to conceive may also choose to undergo fertility testing to establish a baseline understanding of their reproductive health or before pursuing fertility preservation.

Potential Causes of Male Infertility

Male infertility can develop when sperm production, function, or delivery is disrupted by structural, hormonal, genetic, or environmental factors. Commonly identified causes include:

  • Varicocele
  • Azoospermia (no sperm present in the semen; can be obstructive or non-obstructive)
  • Low sperm count (oligospermia)
  • Poor sperm motility (movement) or morphology (shape)
  • Hormonal imbalances (e.g., low testosterone)

What is a semen analysis, and why does it matter?

A semen analysis is often one of the first diagnostic tools used to evaluate male fertility. While relatively simple to perform, it provides extensive information on key aspects of sperm health. 

During the evaluation, a laboratory technician examines the sample for the following:

  • Sperm count: the number of sperm present in the sample
  • Motility: the percentage of sperm that move correctly and effectively 
  • Morphology: whether sperm have a normal structure and appearance
  • Volume: the amount of semen produced during ejaculation

Findings are evaluated against standardized reference ranges set by the World Health Organization (WHO), allowing your provider to determine whether or not any sperm parameters are abnormal and may be impacting fertility. 

Patients are usually instructed to avoid ejaculation for two to five days before collection. Samples may be collected in a private room at the clinic or at home when appropriate, provided collection instructions are followed carefully.

What other male fertility tests might be recommended?

While semen analysis is often the foundation of male fertility testing, additional testing may be necessary when initial findings warrant further evaluation. Additional testing can help physicians investigate why certain abnormalities are present and whether additional factors are affecting reproductive function. 

Medical History and Physical Examination

A fertility evaluation begins with understanding the broader medical context. Your physician may review previous illnesses, surgeries, medications, occupational exposures, family history, and reproductive history. Your physician will also conduct a physical examination to identify any structural abnormalities that may be impacting fertility. 

Hormone Testing

Sperm production is regulated by a cascade of hormones, and if any are out of balance, the effects on fertility can be significant. A blood panel measuring testosterone, FSH (follicle-stimulating hormone), LH (luteinizing hormone), and prolactin can help your provider determine whether a hormonal disruption is contributing to abnormal semen parameters. 

Genetic Testing

In cases where sperm counts are very low or absent, a genetic evaluation may be recommended to investigate whether a hereditary condition is responsible. Karyotyping examines the chromosomal profile for abnormalities such as Klinefelter syndrome, while Y-chromosome microdeletion testing looks for specific gene deletions that are known to impair sperm production.

DNA Fragmentation Testing

Standard semen analysis measures the quantity and movement of sperm, but it does not assess the integrity of the genetic material carried within them. DNA fragmentation testing evaluates the degree to which sperm DNA is damaged or broken — a factor that can affect fertilization, embryo development, and the likelihood of a successful pregnancy, even when other semen parameters appear normal. This test is particularly valuable for men with unexplained infertility, a history of recurrent pregnancy loss, or repeated IVF failures.

Retrograde Semen Analysis

In some men, semen travels backward into the bladder during ejaculation rather than exiting through the urethra — a condition known as retrograde ejaculation. A retrograde semen analysis evaluates a urine sample collected after ejaculation to detect the presence of sperm, confirming whether this condition is present.

Semen Culture

A semen culture tests the semen sample for the presence of bacteria or other infectious organisms that could be affecting sperm function or reproductive tract health. Infections can contribute to inflammation, impaired sperm motility, and other fertility concerns.
