Step 1: Understanding the Two Core Components
For gay male couples, parenthood through assisted reproduction typically requires two key elements working together:

Egg Donation — A carefully screened egg donor provides the eggs that will be fertilized with one or both partners' sperm in the laboratory.

Gestational Surrogacy — A gestational carrier (sometimes called a surrogate) carries the pregnancy to term. She has no genetic connection to the baby, as the embryo was created using donor egg.
These two components can feel like a lot to navigate, but with an experienced fertility team by your side, the process is far more manageable than it may seem at first.
Step 2: Choosing an Egg Donor
Egg donors are typically screened through licensed egg donation agencies or a fertility clinic's in-house program. Donors undergo extensive medical and psychological screening, and you will be given access to a donor profile that includes information about physical characteristics, educational background, personal interests, and health history.

Step 3: Finding a Gestational Carrier
A gestational carrier is a woman who agrees to carry a pregnancy for another family. She undergoes a thorough medical and psychological evaluation before being matched with intended parents. Many couples work with a surrogacy agency to find and vet a carrier; others work with a known surrogate such as a trusted friend or family member.
It is important to have a reproductive attorney involved early in the process to ensure that all legal agreements are properly established before the embryo transfer takes place. Step 4: Creating and Transferring the Embryo
Once your egg donor and gestational carrier are in place, the medical process begins. If you are using fresh donor eggs, the egg donor undergoes ovarian stimulation and egg retrieval, and the retrieved eggs are fertilized in RMG's laboratory using sperm from one or both partners (some couples choose to fertilize eggs with both partners' sperm and make decisions about transfer later, based on embryo quality and genetic testing results). If you are using frozen donor eggs, the donor has already undergone the stimulation procedure, so eggs are thawed before fertilization in the lab.
The resulting embryos are grown in the lab for several days, and our team may recommend preimplantation genetic testing (PGT-A) to assess chromosomal health before transfer. When the carrier is ready and the timing is optimal, the embryo is transferred in a brief, minimally invasive procedure.
Step 5: The Wait — and the Beginning
After the transfer, there is a two-week wait before a pregnancy test confirms whether implantation was successful. If the test is positive, the gestational carrier's care is gradually transition to her OB-GYN, and you will begin preparing to welcome your child into the world.
